Although periodontal (gum) disease usually affects your gums first, your teeth may eventually suffer. That’s because the disease can damage both attaching gum tissues and supporting bone.
One advanced sign of this is when one or more teeth become loose. A loose tooth is an alarm bell that you’re about to lose it.
Fortunately, we can often treat loose diseased teeth with a two-phase approach. First and foremost, we need to bring the gum infection under control by removing plaque and calculus (tartar) — the “fuel” for the infection — from all tooth and gum surfaces. Depending on how extensive it is, we have options: we can use specially designed hand instruments to remove plaque and calculus, ultrasonic equipment that loosens and flushes plaque and calculus away, or, if necessary, conventional or laser surgery.
Depending on the extent of the infection, in some cases we may need to use regenerative surgical techniques like gum and bone grafting to replace lost tissue. Healing takes time, though, which leads to the second phase of treatment — securing the loose tooth during gum healing.
The most common way is through a bite adjustment, where teeth are altered to equilibrate chewing forces evenly. This results in all the teeth being hit at the same time allowing the loose teeth to heal and tighten up.
Another option is splinting teeth together. Although there are different methods, the basic idea is to join the loose teeth with stable teeth like pickets in a fence. One way is to bond splinting material across the back surfaces of the involved teeth. Another way is to cut in a small channel across the teeth and insert and bond a rigid strip of metal to splint the teeth in place.
The splint is usually a temporary measure while the gums heal. In some situations, though, we may need to perform a permanent splint by crowning the affected teeth and then splinting the crowns together. If you have a grinding habit we may also prescribe a night guard to limit the damage done while you sleep.
Before deciding on which technique is best for you, we would first need to evaluate the health of the affected teeth to see whether the effort would be worth it. It could be the tooth’s supporting bone structure has become so deteriorated that it might be better to extract the tooth and consider an implant or other replacement. First, though, we would attempt if at all practical to save the tooth — and the sooner we begin treating it, the better your chances for such an outcome.

Your gums’ primary role is to protect your teeth and keep them firmly in place. But periodontal (gum) disease can damage your gums to such an extent you could ultimately lose your teeth.
Gum disease is a progressive infection caused by bacterial plaque built up on tooth surfaces from poor oral hygiene. The initial infection triggers inflammation, a defensive response of the body characterized by swelling, redness and bleeding of the gums. An initial form of the disease known as gingivitis occurs in most people after just a few days without brushing or flossing.
Resuming hygienic activities to remove daily plaque, along with regular dental cleanings, may be enough to stop gingivitis and restore healthy gums. If the disease is allowed to advance, however, the infected gum tissues will begin to detach from the teeth, turning the slight normal gaps between teeth and gums into wider voids known as periodontal pockets that fill with bacteria leading to infection. Your hygiene efforts will not be enough to cope with this advanced form of periodontal disease.
At this point professional techniques are required to adequately remove plaque and calculus (hardened plaque deposits), depending on the depth and location of the periodontal pockets. The most basic of these is scaling using specialized hand instruments or ultrasonic equipment to remove plaque and calculus in pockets at or just below the gum line. If plaque and calculus have extended to the roots we may then need to employ root planing, in which we “shave” offending material from root surfaces. In some cases this may require accessing the area surgically beneath the gum tissue.
As plaque removal progresses, inflammation will begin to subside and the gum tissues heal. If, however, swelling, bleeding or pus formation persists, this may indicate bacterial levels remain too high. To decrease these levels we may need to administer antibiotics, or through mouthrinses containing chlorhexidine.
Once under control, it’s crucial from then on for you to maintain a strict daily regimen of brushing and flossing to keep plaque from building up on tooth surfaces. You'll also need to visit us regularly (two or more times a year) for professional cleaning and checkups. Keeping a close eye will help prevent a reoccurrence of this serious disease and prolong the life of your teeth.

Are bleeding gums something you should be concerned about? Dear Doctor magazine recently posed that question to Dr. Travis Stork, an emergency room physician and host of the syndicated TV show The Doctors. He answered with two questions of his own: “If you started bleeding from your eyeball, would you seek medical attention?” Needless to say, most everyone would. “So,” he asked, “why is it that when we bleed all the time when we floss that we think it’s no big deal?” As it turns out, that’s an excellent question — and one that’s often misunderstood.
First of all, let’s clarify what we mean by “bleeding all the time.” As many as 90 percent of people occasionally experience bleeding gums when they clean their teeth — particularly if they don’t do it often, or are just starting a flossing routine. But if your gums bleed regularly when you brush or floss, it almost certainly means there’s a problem. Many think bleeding gums is a sign they are brushing too hard; this is possible, but unlikely. It’s much more probable that irritated and bleeding gums are a sign of periodontal (gum) disease.
How common is this malady? According to the U.S. Centers for Disease Control, nearly half of allÂ Americans over age 30 have mild, moderate or severe gum disease — and that number increases to 70.1 percent for those over 65! Periodontal disease can occur when a bacteria-rich biofilm in the mouth (also called plaque) is allowed to build up on tooth and gum surfaces. Plaque causes the gums to become inflamed, as the immune system responds to the bacteria. Eventually, this can cause gum tissue to pull away from the teeth, forming bacteria-filled “pockets” under the gum surface. If left untreated, it can lead to more serious infection, and even tooth loss.
What should you do if your gums bleed regularly when brushing or flossing? The first step is to come in for a thorough examination. In combination with a regular oral exam (and possibly x-rays or other diagnostic tests), a simple (and painless) instrument called a periodontal probe can be used to determine how far any periodontal disease may have progressed. Armed with this information, we can determine the most effective way to fight the battle against gum disease.
Above all, don’t wait too long to come in for an exam! Nearly half of all Americans have some form of periodontal (gum) disease. Without proper daily hygiene and treatment, this aggressive disease can ultimately cause tooth loss. It also appears the effects of gum disease reach beyond the mouth, as researchers have found relationships between it and other systemic diseases.
Inflammation, the body’s response to infection, is at the center of these relationships. In the case of gum disease, periodontal tissues become inflamed as the body attempts to isolate and fight the infection. If the inflammation becomes chronic, however, it will begin to damage gum tissues.
Inflammation is also a major feature of diabetes, a condition in which the pancreas doesn’t produce enough insulin. Without enough of this hormone that transforms sugar into usable energy for the body, the sugar accumulates in the blood stream; as a result, the patient becomes more susceptible to an exaggerated inflammatory response when the body encounters an infection. This is especially true for periodontal infections: the resulting inflammation can be greater and harder to control in diabetic patients. What’s more, uncontrolled gum disease may worsen their blood sugar levels.
Although not as prominent as with diabetes, cardiovascular disease also seems to share a connection with gum disease. This collection of chronic conditions like high blood pressure or restricted blood vessel flow raises the risk of heart attack or stroke. Like gum disease, inflammation is a major component in the progression of cardiovascular disease — in fact, both diseases leave similar chemical “markers” in the blood that indicate their early development.
Ongoing research has also produced some promising treatment findings for both gum disease and inflammatory diseases, which also include osteoporosis, respiratory disease and rheumatoid arthritis. We’re now finding in many cases that treating one side of the disease connection can have positive effects on the other side. For example, diabetics who receive professional treatment for gum disease may see better blood sugar control.
With this in mind, the best approach is to practice effective, daily oral hygiene to reduce the risk of gum disease, coupled with regular office cleanings and checkups. Not only will this help you maintain optimum oral health, it may also contribute to better management of other conditions you may have.

Did you know that roughly 75% of Americans suffer from some sort of gum disease? Gum disease (also known as periodontal disease) refers to the progressive loss of attachment between the fibers that connect the bone and gum tissues to the teeth, and the consequential loss of the tooth-supporting bone itself.
As you get older, your chances for developing gum disease increase significantly. Here are a few other factors that might put you at a higher risk for developing gum disease:
- Family History. 30% of the population may be genetically susceptible to gum disease. Luckily, there are new tests that can assess if you are at risk. However, even with good oral hygiene, studies have shown that genetically susceptible individuals may be 6 times more likely to develop gum disease.
- Tobacco. Smokers are four times more likely to develop periodontal disease. Smokers have more disease-causing biofilm bacteria and collect it more quickly. They also lose more attachment between the teeth and gums, which leads to more loss of bone that supports the teeth.
- Bleeding Gums. Some people mistakenly think that it is normal for their gums to bleed when they brush. In fact, this is an indication that you are not brushing and flossing effectively and a common symptom of gum disease. Pregnant women are also more likely to have bleeding gums, because elevated hormone levels may cause gum tissues to be more responsive to bacterial biofilm, thus bleeding more easily. That is why we recommend that pregnant women have regular cleanings at three to four month intervals.
We often refer to gum disease as silent, because symptoms may not appear until the disease has advanced. For this reason, you should conduct a self-assessment to identify if you are at risk.
- Have your gums receded and/or do your teeth appear longer?
- Are any of your teeth feeling or getting loose?
- Do your gums appear red or swollen?
- Have you recently had a tooth or teeth extracted because they were loose?
